The muscle over the hip bones, usually firm and rounded, felt thinner. Almost hollow. He told himself it was probably nothing — Remy was still eating, still greeting him at the door, still hauling himself up onto the couch he wasn’t supposed to be on. But that one small detail stuck with him enough that he mentioned it at Remy’s next checkup.
His vet didn’t dismiss it. She asked him to describe exactly where the thinning was, how long he’d noticed it, and whether Remy’s back legs seemed to wobble more on hardwood floors. That conversation turned into a full neurological and orthopedic workup — and it turned out Dave had caught the very earliest physical sign of a condition that, left unnoticed, often isn’t diagnosed until a dog can barely stand.
Why Muscle Loss Hides So Well in This Breed
German shepherds are built with a lot of muscle mass to begin with, especially through the hindquarters and topline. That’s part of what makes early muscle wasting so easy to miss — a little bit of thinning on an already muscular dog doesn’t look dramatic. It looks like a slightly leaner version of the same dog. Owners chalk it up to age, weight loss, or a slower metabolism, and life goes on.
But in this breed specifically, hindquarter muscle loss is one of the earliest visible clues of degenerative myelopathy, a progressive spinal cord disease that German shepherds are genetically overrepresented in. The disease starts in the spinal cord, not the muscles themselves — nerve signals to the hind legs gradually weaken, and the muscles atrophy simply from disuse, long before a dog shows obvious wobbling, dragging, or knuckling of the paws. By the time those more recognizable symptoms appear, the muscle loss underneath has usually been building for months.
It’s Not Always Neurological — And That Matters
Muscle wasting in German shepherds doesn’t only point toward the spine. This breed is also disproportionately affected by exocrine pancreatic insufficiency, a condition where the pancreas stops producing enough digestive enzymes. Dogs with EPI can eat enormous amounts of food and still lose muscle, because nutrients simply pass through them unused. The tell here is often a dog that’s ravenous, thin, and losing muscle tone despite a hearty appetite — a very different pattern from the general slowing-down of degenerative myelopathy, but one that gets missed just as often because owners assume a hungry dog can’t possibly be malnourished.
Then there’s the quieter, more universal cause: disuse atrophy from arthritis or hip dysplasia, both of which German shepherds are also prone to. A dog that’s subtly favoring a hip or avoiding stairs will lose muscle in that limb simply from not using it fully, even if the discomfort itself is mild enough that the dog never limps in any obvious way.
What Vets Actually Look For
Veterinarians use something called a muscle condition score, run alongside the more familiar body condition score, and the two aren’t the same thing. A dog can carry a completely normal amount of body fat while still losing significant muscle mass — which is exactly why muscle loss slips past owners who are only checking whether their dog looks “too thin” in the general sense. Muscle scoring focuses specifically on the temporal area of the skull, the shoulder blades, and the hips, feeling for the loss of definition under the fingers rather than looking at overall silhouette.
When muscle loss is confirmed, the workup usually branches in a few directions at once: bloodwork and sometimes a specific fecal test to rule out EPI, orthopedic evaluation for joint disease, and in dogs over seven or eight, genetic testing or a neurological exam to check for degenerative myelopathy. Genetic testing for the SOD1 gene mutation associated with the disease is now widely available and can flag risk even before any symptoms appear.
What Early Catching Actually Buys
For Remy, an MRI confirmed early spinal cord changes consistent with degenerative myelopathy. There’s no cure for it, and that’s a hard thing for any owner to sit with. But catching it at the muscle-thinning stage, rather than the stumbling stage, meant Dave’s vet could start physical therapy and a structured exercise plan immediately — the kind of intervention that research increasingly suggests can slow the muscle loss and extend the time a dog stays mobile and comfortable.
That’s really the point of paying attention to something as unglamorous as how a dog’s hips feel under a towel. Muscle loss in German shepherds is rarely loud. It doesn’t announce itself with a limp or a dramatic change in behavior. It shows up first as a quiet difference under the hands of someone who touches that dog every single day — which means, more often than any test or scan, the earliest diagnostic tool is simply a familiar owner paying close attention.
